Referring patients can often be a timely process for physician offices – our goal is to make it as easy as possible for all involved! To refer a new patient to Body One, simply complete and submit the new patient referral form included on this page. Our office staff will respond within 24 hours by contacting the patient, scheduling an appointment and verifying their insurance benefits. Should we happen to encounter difficulties in reaching the patient, we will be in communication with your staff to quickly resolve the situation.
As an additional option, a PDF version of our referral form is provided, should you prefer to fax or email patient info to one of the clinic locations listed below.
Our therapists will provide a detailed evaluation and plan of care, followed by timely progress notes to keep you informed of the patient’s status. We welcome questions and discussion to ensure the best possible outcome.
